Enrollment in Lamphere Schools is based on residency, which is where a parent/legal guardian owns/rents and physically resides.  Within our district boundaries, your residency address is assigned to a specific attendance area/elementary school. Residents of Lamphere Schools and approved Schools of Choice applications please complete Online Pre-Enrollment. Documents uploaded should be in PDF format and legible. We may require in person documentation if the proofs uploaded online are not clear.

Enrollment Requirements

Required Documentation to complete Enrollment:

  1. Original Birth Certificate with Seal (demonstrating parentage)

  2. Parent/Guardian Identification (Valid Driver's license /State ID/Passport showing photo identification of parent/guardian name)

  3. Legal Guardian Court Order - if applicable

  4. Custody documents - if applicable (Please note that unless the court order specifies otherwise, Lamphere Schools is required by law to permit custodial and non-custodial parents/guardians the same access to their student and the student records. )

  5. Immunization Records (up to date) – For more information on Immunization Requirements

  6. Transcript of grades from previous school (students in grades 9-12) -OR- Report card from previous school (grades K-8)

  7. Special Education Documents – Current IEP/MET/504 (for students receiving special education services)

  8. Vision Screening (Jr. Kindergarten and Kindergarten ONLY)  Vision Screening Requirements

  11. Residency Documentation and Verification – Provide the required documents outlined below for your residency.  If you move to another address, it is your responsibility to notify us of your new address.

A Qualifying Residence: Is defined as a residential dwelling that is either a single family principal residence as recorded with the Michigan Treasury or a legal single family rental unit that meets the Michigan Treasury and local community requirements for a rental. The Lamphere Schools, Madison Heights, MI will participate in unlimited Schools of Choice for the 2025/2026 school year for students (JK - 12) who reside in Oakland County Schools Districts other than Lamphere Schools and contiguous counties.

Our application period will run March 31 through August 13, 2025.

State and Federal Requirements

When enrolling a student in any Michigan school district, the State School Aid Act, related Michigan Compiled Laws and other federal and state requirements apply. Each district is responsible for verifying three things: parentage, a qualifying residence and residency in the qualifying residence.
